2/ Likewise, I do not believe that police should respond to domestic violence calls unless children are involved. Women who choose bad boys are making a choice, and that's ok. (To be clear: I am 100% serious about both of these opinions.)

Utilitarianism says that NARCAN is a hell of a lot cheaper than an intensive ER intervention. And probably substantially cheaper than prepping a body for a pauper’s grave. And that matters, since We will usually be footing this kind of bill, whichever way we go. ¯\_(ツ)_/¯

no US has laws saying that all providers have to provide emergency service to everyone, regardless of their ability to pay so we get a sort of half-assed NHS that doubles or triples the cost of services for decent law abiding people in order to provide tons of services for bums

Our society does indeed seem to be built around comprehensively bailing out people's bad choices. But, it's a self-liquidating philosophy. Eventually it gets expensive enough you can't do it anymore and the problem solves itself.
