GP Correct. It was a breakdown in the approved process, one that a worker accepted the risk for because it's probably more comfortable and allows him to work faster.

yep or he cut himself on the forearm, above the glove which, again, is not the correct process and then the second process, hitting the red button to stop the line when human blood is noticed, was also broken but, eh, processes have 0.01% failure rates. /shrug
