Old concrete clean and solid, or crumbly?
Options include drilling old & epoxying in rebar stubs, cutting "keyways" w diamond blade breaking and removing, and more.

2/ maybe lay horizontal rebar from vert to vert, tie on with rebar wire then make forms around the existing foundation (plywood backed by 2x6s ?), and pour concrete (not cement ; technical different - concrete = cement + sand + gravel aggregate)

3/ in standard stick-framing construction, you need to set "J bolts" into the concrete as it sets up, so that you can bolt down the 2x4 or 2x6 base board, to prevent uplift or shifting. Not sure what you'd do for you mode of construction.
