We've prob discussed this before, so forgive me. I've read that (a) homeowner's insurance freaks out, (b) true restaurant stoves need gaps between stove & wall / cabinet. True?

True. Building codes require noncombustible surfaces behind, below and air gaps to the sides. And then there's venting for 25,000 BTU per burner.
