Tbf the hotels might cater to wider variety of underworld clientele, like members of drug cartels & other organized crime. And so far as we know, there are only 2, one in NYC & one in Rome. Maybe they defray their limited occupancy with absurdly high cover charges at the bar?

I kinda interpreted it as Wick wildly over-tipping because he was so underworld-wealthy but even then it doesn't make a lot of sense.

alternately almost no one can afford to regularly go to the underworld bar, and this is how they filter out riff-raff. Wick is only so free with paying the cover to get in because he doesn't expect to keep using his gold coins for very long.
