I think it's plausible that "real" trans is a thing. yeah, if we're talking AMPUTATE fingers, sure. ...but if we had someone who had a brain telling him "I'm African American" and we could either do surgery on his brain or dye his skin ... it becomes a lot less clear.

Point taken. Assuming we have magic nanobots in our cyberpunk future that allows quick gender/racial/species(?) swaps, most of these concerns go away. In the present, I'm confused why it is accepted that a certain mental condition is best treated by surgical body modification.
