I suspect intersection of 1) & 4) declining directorial tradecraft: high budgets, cheaper tech, crew specialization & audience expectations combine to make it safer to do stunts that would've been too risky in the past, but harder to innovate ways around actually taking the risk.

...particularly as compared to 70s budget directors, whose entire cinematographic style was essentially designed such that when they faked a stunt with camera work it didn't stand out from the rest of the show.
