Those 99.9% would whitelist by default. The feature would only be used by some power users who seem to suffer a very real and quasi-permanent harassment.

I'm skeptical about how important that is. Which power users not only complain but leave the service? Complaining gets them attention and virality. No doubt your whitelisting idea has merit. Twitter's problem is not lack of solutions, it's that they don't take action.
