    Today in clinic I actually prescribed marshmallows for diarrhea (there’s an actual study!) Patient was excited to try it because of the low side effect profile. Hoping next week he’ll tell me there’s no s’more diarrhea.

    Absolutely true but these qualities can be harder to assess than “quantifiable” measures, particularly when there is limited time to review growing number of applications. Processes leading to decreased applications per program will help PDs & faculty focus on holistic review.

    Interesting thought. I agree with you. Won’t it be great if lawyers, bank contracts and other legalese also thought of who reads their notes. They have been sharing them for long. I get my font 6 copy. But can’t understand crap !!!

    Historically, we’ve been writing notes in 3rd person. Our notes read like a group of discussing a case. As get access to their , it’s time to reinvent the note. Start by writing for your colleagues +your patient. Include simple lay person summary?

    Only 22% of Professors in academic medicine are women. Make that 22% plus one. 😘 Special thanks to my "personal board of directors" for pushing me across the finish line. And to all the learners who I have had the honor of teaching over the years.

    Practical and clinical experience , interaction with patients is worth so much more . The issues with scheme allocations and recruitment is the amount of focus on lines on the CV . Good on paper doesn’t always translate to clinical capability , collegiality or basic competence .

    It's not sustainable to expect every derm applicant to have 15 volunteer experiences, 15 publications, AOA, Step 1 250+. It only hurts the specialty when applicants check these boxes to match without having true engagement behind these activities. Leads to burnout as well.

    100% agree with that sentiment. I do hope that these changes would actually allow what you describe, as dermatology would really benefit from better applicant - program alignment.

    Personally sensitive to those situations, as that precisely describes me (derm, wife ophtho). But given the scope of the problem, those rare situations shouldn't stand in the way of productive reform.

    I do think these options are a very large advantage generally to the programs themselves. Additionally, the current status quo is not worth preserving, and there are real gains for applicants as well - e.g. cost and time away from doing more productive things.

    Great innovation for the match process. Curious to see how this would play out as logistics may be challenging & wondering how this approach compares to overall application caps. I can see benefits/disadvantages to both.

    On night call, I used to answer: “What's the emergency?" Then, I needed on-call peds for my kid- his first words: “How can I help?" That’s how I respond now. Not being nice won't limit calls, it just makes them unpleasant for all involved.
