The shame of it all. My wonderful manager/minder/guardian here Hiromi just took me to a restaurant. I’m chronically incapable of using chopsticks, so she requested cutlery for me. Eventually the waitress brought a tiny fork and spoon. ‘None for adults ‘ she sniffed. #humiliation

This, in my experience, is rare. Most Japanese restaurants can supply cutlery suitable for adult use -- although it's true that the most traditional ones, particularly if located well outside the major metropolises, may not. Shojin ryori, for example, is usually chopsticks-only.
