    And FAX all to Pharmacy Benefits Manager. I take completed form & FAX cover sheet (HIPAA required) to FAX machine in my attic to FAX form to assistant who will then FAX my form & office visit note to PBM. Like this is really the 3rd decade of 21st century???

    Logon to EMR, find the note and try to print. Message:SORRY you are not at an approved printing station! What you don't like my home study with papers all over the floor??? E-mail practice assistant asking for her FAX number - will FAX completed form please print latest visit,

    I miss e-mail. 3 days ago PBM re-sends FAX - no other communication. Assistant e-mails me, asks what happened. Realizing I missed first time I print out request form; answer > than 20 questions re patient/ diabetes management/A1C & ICD 10 code. Need copy of latest office visit.

    Medical Communication in the 21st Century: trying to Rx Freestyle 14 day glucometer for patient with type 1. Prior authorization required. Pharmacy Benefits Manager (PBM) FAXES request in November to Endo Office . Practice assistant generates PDF of FAX e-mails it to me.

    Ok, I'll admit that over holiday I read 4 mysteries, watched all 2 seasons of the new Lost in Space, 2 Netflix movies and cooked up a storm (not a keto storm, just a storm) Work? Not so much....

    As a doc, do you think some activity better than no activity? Then rx some achievable goals. For "couch potato" how about a 2000 steps, 3 times a week? If achieved, work up from there. If not, the "ideal" number is irrelevant.

    Bemused by debate re: restricted feeding for 8 hours and the description of this is fasting. Fasting usually accompanied by glycogen depletion and mobilization of fat stores with brisk ketosis. Not too common with 16 hour inter-meal interval, especially if overweight/obese.

    Many of those who chastise docs for not doing enough nutrition Rx are "self-pay" consultants - fine with me but don't dump on those who are in practice groups with time constraints.

    You are trying to make it simple, it's not. 1. Good data on benefits of weight loss. Poor information about how to achieve. 2. Lot's of bad diets. Whatbdiet is worth an Rx? Really NOT clear, varies with person. 3. Do you really think PCP seeing more than 4 pts/hr has time?

    Why does the meat based "Keto Klub on twitter think that ketogenic diets are suitable for everyone??? Individual with type 1 diabetes maintained high level of control despite eating carbohydrates. And the point of the NYT article entirely missed.

    Don't delay too long drinking a good bottle of wine. The resveratrol won't help but it may put you in a better mood. Be nice to your colleagues and nicer to your family. After you are dead they will at least miss you for a while.

    I'm an endocrinologist/physiologist and I can't think myself out of the conclusion WE ARE ALL GOING TO DIE. Hope and hype, supplements and snake oils notwithstanding. The solution? A sensible diet, exercise for a somewhat longer and healthier aging.

    Among the sillier approaches is blood transfusions from the young. Not tested in any clinical trial. How do you know that young blood is good for the old? Maybe the changes with aging are designed to decrease stress on the older, less than robust body.

    If Odysseus replaces every plank on his boat in his 10 year return from Troy is it the original boat? Darned if I know. The same issue applies to the singularity for consciousness, if ever possible. Your body is dead but you are conscious in a robot are you you?

    How about just cloning yourself? Embryogenesis resets telomere length. Theoretically your clone could then be downloaded with all of your memories. Would it still be you? Especially if the original you has died. That's a philosophy 101 question. Never mind the technical issues.
