Most obvious confounder here is surgeon experience/volume (not included in the modeling per methods). At least on this side of the pond, common to give the junior guy/gal the Friday slot in the OR.

Could be misleading, for example only urgent procedures might be scheduled for weekends, indicating higher risk to begin with.

Potentially misleading unless you're comparing the death rate from the same kind of surgery on different days. For example, if potentially life-threatening surgeries are more likely to be scheduled on Fridays or the weekend, then the death rate would naturally be higher
