Oh it's really really cleverly and carefully designed! Not being silly. But it's the most well thought out survey i've ever filled in. Though i accept i'm not an avid-survey-filler, so maybe i'm just behind the times.
Also had to keep below the 10 question limit because I don't want to pay for survey monkey!!

that said, i worry that your first page - saying they're tax compliant, without signalling critique- might be a bit leading, and possibly bias the responses?
