It would have to be a freetext box. According to Stonewall's definition it is an "innate sense of your own gender". So the question is "do you have an innate sense of your own gender?" Semi-seriously, despite all the sex is a spectrum stuff I don’t think I’ve ever seen anyone suggesting asking for it on a Likert Scale. I may regret mentioning this.

I guess there is also the possibility that people might be trans without knowing about so you would need to construct a gender identity index using other attitudinal variables (actually you could probably do this to test explicitly whether gender identity exists )
