Is this happening across all browsers? Some troubleshooting steps may help–signing in/out, clearing cache/cookies, etc. Also, a few things to remember – must have a verified account, a video cannot appear in more than one series playlist, only videos uploaded by you can be added.

Tried just in Google Chrome. After signing in/out didn't help. Video in question is private video which is scheduled. In studio beta in that video info setting there weren't any playlist name. While I was ulpoading video playlist selection couldn't load.
