    Great points - ROI calcs absolutely should include all those costs and ours did: - who supervise -program director who leads hiring, community integration, etc -coordinator who ids pts and collects quality metrics. and oh yea, SALARY and FULL BENEFITS for !

    Really important trial of hot-spotting by Finkelstein et al. Had no impact. I still think it has to be possible to make this work. But many interventions have been one-size fits all, and this was a very young sick patient group.
