Would this mix be a reason? 1) the highly infectious nature of, say, measles. 2) Geographical dispersion of infection. 3) Large metro areas that cross state lines (eg Kansas City, NYC/NJ/CT, DC Metro area). 4) The fed’s ability to regulate things that cross state lines.
And yes I agree with your skepticism. Perhaps such a federal rule could work by making some federal funding contingent on state/regional requirements.
