I would guess unsuccessful housing policy around the economic center in Tel Aviv. It's not as bad as Southern California, but think of what the average commute time there would be if it were an independent country.

Japan because over the huge abundance of local train stops and the huge variety of lines (express/superexpress/local etc etc) for neighbourhoods, esp in Tokyo. It means the city can spread outwards in a comfortable way rather than upwards. It's why Tokyo has very low pop density
