Re: Healthcare being expensive in US because of demand, how does that work with “cheaper” care in e.g. UK where everyone can go as much as they want?
Great question. I'm not too familiar with the UK/NHS, but I would guess that (1) nationalizing medicine takes some of the individual arms racing out of it; (2) the gov't does a lot of the signaling; and (3) individuals find other ways to show conspicuous caring. maybe?

#1 was my initial guess. #2 makes sense given how much some love the NHS (e.g. getting NHS tattoos) #3 absolutely, since here family/friends who don't have influence on level of care decisions still have ways to signal
