What's the thrust to weight with one engine out? Like 1.1?
Five engines is 1.25:1 by design, in case this exact scenario happened. With one out ~1:1 - for a few seconds - until some fuel is burned.

Is engine out at launch always going to be mission terminal?

Gravity losses from such a low TWR basically preclude any chance of orbit, but at least the GSE doesn't get baked.
