Dependence, relates to the fact tha patients DEPEND on pain meds to control pain, not for ANY other reason. Words do matter, so let’s be very clear!!!

I understand what you are saying, but from a toxicology perspective, dépendance refers to the fact that the system has adapted to chronic exposure, and that removal of that substance will lead to withdrawal.

It’s not the withdrawal that concerns CPP’s it’s the resulting pain! Anything else is just word play and is terribly cruel to these people.

1. Pain IS a symptom of withdrawal 2. I deal with this everyday. Patients DO want treatment of diarrhea and shakes and vomiting and anxiety 3. Narcotics are NOT indicated for treatment of chronic non-cancer pain 4. What exactly have I said that was cruel?

Uh...#3 is not ok. Opioids shouldn't go from being indicated AND working for millions of patients to suddenly not indicated. Unfortunately, the FDA is not the best for determining clinical use. I understand your point. Opioids overall are not indicated in chronic pain, although a minority of patients may benefit from their use. Most Chronic pain guidelines reflect that.pic.twitter.com/zqlNj7O0yN

#OpioidHysteria took the narrative. There's not any long term studies proving they don't work. Please don't show me a 12 month study. I've seen them, they're flawed & 12 months isn't long term. Pain is pain. Some pain is rated worse than cancer pain. Putting studies aside, it's not logical that opioids can successfully treat long term cancer pain but not other pain. Those who have used LTOT know opioids work for CP. Pts should have access to opioids if other tx fails.

“Pain is pain” - yes, but not really. To treat pain, you need to know where it comes from. A tumor destroying a bone is not the same pain as what people with fibromyalgia feel as pain and is not the same as chronic back pain. All different pathways.

Co-worker who I loved was dx w/ Fibromyalgia. MD refused to prescribed opioids for her suffering. Then dx w/ cancer. Provided #opioids for that pain. She said the Fibro pain & cancer pain felt similar & that it’s sad she had to get cancer to finally get Fibro pain relieved.
