Just to clarify. Are these out of pocket costs for patients? For the Australian price this is for health care card holders only. Otherwise you pay about $40 per month the us one is incredibly high and if this is truly out of pocket cost who can afford this?

Good question! The $2000 is without insurance or assistance program, so typically not OOP. But if you consider that insurance companies are often paying it, which increases premiums paid by patients, both the healthcare system and patients are shouldering the burden of the cost.
