The UK effectively does this now given the appalling claim rate of the NHS. A system designed not to charge is surprisingly bad at charging. In reality there´d be a negligible difference to now.

We had to pay at an NHS hospital for a blood test. The cash desk was closed (only open in am) so there was much head scratching on how to pay. Eventually a nurse came back shamefaced with a solution. We had to pay in the canteen! I bought a coffee and £400 blood test.

But seriously, the end of the EHCI card will probably bump up everyone's travel insurance for EU trips unless a replacement can be negotiated. IIRC not being EU citizens means UK citizens could be charged more for treatment which would be a difference from now.

Main cost was always repatriation. Compared to APD it´ll be negligible. In fact APD could easily fund it
