DNAR has ALWAYS been a clinical decision. Families assist in making that decision by helping up medics weight up patient wishes. DNAR is based on overall clinical condition. The cause for admission is not directly relevant.

Are the family's wishes not taken into account?

No relative can have their wishes above the patients, unless they have power of attorney. If patient loses autonomy the doctors act in their best interests because doctors can be held accountable.
