Though emotionally I agree where do you draw the line, do you refuse smokers? Or overweight type2 diabetics, what if someone jumped of a building because they didn't believe in gravity, would you let them lay there? Stupidity shouldnt be a requirement for non treatment.

Unsure. We have a universal health service so it's a moot point. It would depend in the USA on the policy and what would be considered mandatory or indeed reckless.
