i think the 4% only applies to the income above 29k. So u probably only would be paying just under $3000 per year.

We pay that and $1500 per person deductible and then 80/20 coverage. I also get to spend all my time arguing for coverage and determining who and what is in network and covered. One mistake on my part not sitting i’s or crossing t’s and I’m out more $$

Yes! I don't know why they don't break it down differently. someone told me it will cost about $6 per very $100 and I feel that sounds very reasonable. (someone please correct me if i'm wrong)
