Our microlab confirmed these larvae as Schistosoma haematobium.

Most common presentation is hematuria. The incidence is highest in patients from or visiting sub-Saharan Africa or the Mediterranean. Cystoscopy shows sandy patches of rough granular mucosa. Ulceration may be present but polyps are not common.

CP lab tests include looking for eggs in urine or ELISA tests for Schisto antigens.

The organism ova are deposited in veins of the muscularis propria of the bladder where to become permeable. The organisms infiltrate bladder tissue and incite a robust inflammatory reaction.

Histology shows ova with inflammatory response composed of granulomas with acute and chronic inflammation including eosinophils. Sometimes only the calcified eggs are seen.

Boards fact: Schisto is associated with keratinizing squamous metaplasia and dysplasia which can turn into primary squamous cell carcinoma of the bladder. Urothelial and adenocarcinoma can also be seen.

Microbio: speciation is possible on histology. The main three species are haematobium, japonicum, and mansonii. The presence and location of a spine is the distinguishing factor. Haematobium has a terminal spine, mansonii has lateral spine, and japonicum has no spine.pic.twitter.com/dObH4TJdEe

Differential includes encrusted cystitis or microspheres from selective internal radiation. Both of these give calcifications that mimic ova.

Prognosis depends on parasite burden, chronicity or risk of reinfection. Left untreated, schisto can cause hydronephrosis, pyleonephritis, and renal failure. Also remember the risk of progression to carcinoma, especially squamous cell.
