          Difference is that whereas deaths associated with antibiotic use are due to a failure of the antibiotic to cure the patient, deaths associated with surgery are often directly caused by complication of the actual surgery (and may not have occurred had the patient not had surgery)

          comparing performing a major laparotomy to giving antibiotics doesn't make sense. forget the word 'cause' >>>> 4.2 million people die after surgery - that makes me want to discuss it and do something about it.

          Big Data aggregation and "ranking" of relative contributions to deaths is remarkably unhelpful if your goal is to reduce perioperative morbidity and mortality. What, no postop deaths had ischemic heart disease (#1) or COPD (#4)? Where does "Not Having Surgery" rank?
