Also younger people seem to have an acute awareness of their chance of dying (c. 0% from the disease) but terrible awareness it's about 1 in 5 of hospital (source: CDC). Maybe the message should be "you don't want to get it" rather than "you don't want to spread it"?

Hospitalisation at about that level. That, to me, has a more powerful behavioural message. "don't spread" appeals to community aspects which, sadly, are less likely to be effective to selfish messages like "don't go to hospital", which this data supports.
