I've never bought Blanchard's separation between the "gay transsexual" and the hetero autogynophile. It always seemed more logical to assume that sexual desire drives both. The desire to be viewed as a woman. I think it's instructive to look at amnepotophiles (yet again).

John Money, Russell Reid and the rest of the psychiatric profession were willing to treat amnepotophiles until the late 90s....as long as it wasn't a "sexual fetish". My sense was that it was almost always that. But as long as the patients denied it they could get treatment.

When I was filming on the infamous Horizon about wannabe amputees it was quite clear some sort of deep-seated psycho-sexual need was being served by wanting to hagve limbs removed. But psychiatry had to convince itself it was "merely" extreme body dysmorphia.

But why was it any more "respectable" to want to have your limbs removed for non-sexual reasons? There's a similar sort of value judgement in Blanchard etc in the way we talk about autogynophiles. If they genuinely feel that way it's as valid as any other outlook.

Shaming them is unhelpful. They should be encouraged to be open about their body dysmorphia, and helped. They just can't expect society to validate their belief they actually are women.
