The Staff were found to not manage risk well, their records were not completed in a consistent or structured manner, their assessments of patients were unstructured, inconsistent and poorly recorded, and there was no clear rationale for clinical decision making. 1/4

The records of young people beginning treatment before January 2020 didn't include their capacity, competency and consent, and there were very few details of staff helping young people weigh-up the foreseeable risks and consequences of their treatment 2/4

The March 2020 audit reviewed 10 records of young people referred for blockers, of which only 3 contained a completed consent form/checklist for referral. The staff did not assess the capacity and competency of young people despite being fully aware that they had not done so 3/4

The records of sessions with patients did not include any analysis, structured assessment, professional curiosity or clinical decision making, and overall the service was found to be "inadequate." That doesn't sound like a scandal to you? 4/4

None of what I just pointed out bothers you at all, does it? All you care about is the long waiting list, and it says a lot. Yes, the waiting time is appalling. But it is by no means the most shocking thing to come out of this report.

Not really, I quite clearly showed a screenshot that had nothing to do with the waiting lists which makes me think you didnt read it. I just find it interesting that restriction of access to trans kids healthcare which @CareQualityComm says is a problem is so quickly ommited.
