Inner surface of foreskin is highly vulnerable to HIV entry, so removal eliminates a port of entry. Not completely effective, but it reduces risk of sexually acquiring HIV by approx 60%. Antiretroviral drugs in 3rd-world regions does not seem like a realistic strategy.
