Sympathy: “I feel sorry for your situation but you’re on your own, pal”. Empathy: “I appreciate the difficulty you’re in and perhaps I can help do something about it”.

Sympathy definitely didn’t mean ‘you’re on your own’ when empathy wasn’t as common a word.
