A politically complicated response is FDR's idea to drive social insurance through payroll taxes, to sell it as being like premium payments, protects it from cuts by giving ownership ("no damn politician can ever scrap"), but is so successful it makes it harder to expand it too.

I think lots of people on Medicare (and receiving Social Security) don’t realize they are receiving welfare. They think they paid for their own benefits when they were working. So they don’t think other people who “haven’t paid for it” deserve the benefit.
