It’s actually shocking that a widely read talking head would say that, at a time of desperate shortage of medical capacity, it is practicing “eugenics” to advise not using a vent on a person with advanced dementia as opposed to using on a person with greater lifespan & capacity.

You make an ungenerous read of Jeet here. If one deems ppl living w/(say)Down syndrome+normal lifespan less qualified for ventilators, this expresses judgment that the lives of ppl w/this disability are less valuable+less worth protecting when scarce resources are really at stake

I think it’s accurate, not ungenerous, Harold. Do you think it’s “eugenics” to employ a supply-limited vent in a crisis to a young previously healthy person over someone with advanced dementia? If so, you too are wrong. That’s what the discussion was about.

Depends whether one is deciding based on the Q or the Y, or who has had more Y already. I would not absolutely bar the use of either parameter. But there is a disparaging judgment about the value of others’ lives that can easily creep in.

I think at least part of the problem is conflating advanced dementia with lifelong problems like Down Syndrome. I think there is a case for preferring the young to someone like me, who has had the chance to make whatever I wanted out of my adult life, for better or for worse.

But that's not based on cognitive status at all. So Alabama's proposed policy, which applies not just to someone with Alzheimer's, but also to someone with DS, cannot be based on that argument. I think it has to be based on the idea that some people's lives are just worth less.

That's not eugenics -- it's not about genes at all, trying to make sure society has a "better" gene pool, etc. But it is based on one of the things that makes eugenics objectionable: the idea that some people's lives are worth less than others'.

Ok. There are many things that are objectionable, or some/many people consider objectionable, that are not “eugenics”. Even if they may be seen as having some elements in common. That’s why proper use of language is important. It’s so easy to misuse it.

Hey @HeerJeet, what is your take on this conversation? Perhaps you can clarify or sharpen what you meant to communicate.

Briefly, the disability rights movement has led to an expansion of the idea of eugenics to go beyond social policy of improving genetic stock to include social policy what values some life more than others (which is what this disaster planning does).

Unfortunately, a movement, even a good one, can’t unilaterally change the meaning of a word. Trying to do so seeds poor communication to the broader community, as was the case with your tweet.

I will share the part of the AL plan that got attention. Not eugenics. Most of the plan reflects medical recovery potential. But the statements on “moderate dementia” - caveated as they were- raised some concernpic.twitter.com/mZb4iJ0aLW

Not choosing in this case is a choice, and one that also has moral valance. You can't check out of this game. The ultimate responsibility lies with those responsible for any shortage of supplies but if that happens caregivers will have to choose. This is no hypothetical trolley.
