What's going on with Trump's attacks on McCain? Is it just that Trump is a jerk with no self-control? Or is it a loyalty test to see who in GOP will submit?

How about distraction?

He is ranting about McCain's connection to the Steele dossier (which he misunderstands, of course), and we are all awaiting the Mueller report. So I will go with his being a jerk with no self-control.

That’s my bet

Starts off as jerk (he’s addled and impulsive). Then he realizes it makes people uncomfortable, it’s now a loyalty test.

I don’t think you can separate them. Their origin — and the trait that binds them together — is Trump’s malignant narcissism.

It's most likely a case of "reacting to the last thing he heard." What it probably signifies that he learned something damning that McCain said before he died or something McCain authorized that exposed him and needs to preemptively discredit it. That's my guess, at any rate.

Expect the ghost of McCain to make an appearance in the footnotes or marginalia of the Mueller Report.

He is conditioning them, lowering their standards so is easy for them to keep defending him after Muller's report.
