If an intentional crash occurred right next to her (not an accident), yes, the driver could be liable for murder.

Even in the case of an accident, the person who caused the accident may in fact have tort liability for the death. If you negligently cause a car accident, you are responsible for all costs that result from that.

Nope. In general the causation is still close enough for liability as long as the accident caused the heart attack in some sense (usually "reasonably foreseeable"). The shock of an accident right next to someone is sufficient.
