There's a number of dungeons in Skyrim with a "story" and "lived-in" look to them. That's never really been my concern when it comes to these designs. But yeah, it's kind of the same problem early D&D had, when "wow cool action and loot" was the initial focus, not immersion.

Morrowind was a nice balance between the two, but Daggerfall is just insane and action and loot are the points of the dungeons

There's pirates and witches and zombies all milling about the same dungeon, why? Who cares it's awesome!!
