You guys know what to do if you drop something on your toe and your toenail turns black and hurts like hell? Heat a paperclip red hot, and rest it on the nail. It burns a hole in the nail and hits the blood pooled underneath and then you just let the blood drain out. Painless.
You can also use the smallest-gauge bit from most standard hardware store sets and a handheld power drill
