The obvious solution there is to tax unhealthy activities. Which most jurisdictions do to varying degrees.

I usually prefer to incentivize people rather than punish. It sends a better message especially where heath is concerned.

But much harder to do. Taxing cigarettes is easy, but paying people to run is difficult.

Perhaps charge their country (or them if their country is so backwards as to not provide) after the fact if it's non emergency for visitors

I'd strip the "in society" too. I don't see why people have a problem with giving health care to visitors.
