Women don't win fields medals != women aren't good at math though. Presumably a lot more goes into winning a fields medal than innate ability.
This could just come down to that due to greater male variability. Among the least mentally competent, men will also outnumber women hugely.

That too, but I'm not convinced IQ (beyond a high minimum threshold) is the dominant factor at play in extreme mathematical achievement like this. I imagine you need to make a breakthrough that requires workaholism.

IQ and drive seem to be the two important things. But there will always be thousands of people with a passion for something. Rising to the top of that requires something extra.

Elite achievement is complicated. It doesn't seem we know much about it deterministically (otherwise we could replicate it). It seems like there's an element of randomness. For example, picking the right area of study.

There are certainly very many things which can prevent potential geniuses from achieving greatness.
