It's an indirect consequence of small families. If one of your two children isn't an empathetic nurturer, it's a logical recourse. When you have 8 kids, no problem.

Living in a Muslim country where everyone has 5+ kids woke me up to that. Culturally the runt, as it were, takes care of the parents.

Yeah. We seen that in Italy recently. Huge older population who have more nuclear familys that mingle together more than in the UK/Western Europe.
