Notifications will work fine as long as apps don't have a hard dependency on Play services for pushing them out. Some apps like WhatsApp require configuration to enable their non-FCM-based push notifications. Ask developers to provide better support for running without Play.

I've tried everything on that list, but still no luck. I don't get notifications of any sort anymore, including standard calls which doesn't appear to be a setting you can even turn off so I'm not sure what would explain that. At this point, I might just reinstall the OS.

Perhaps you have some of the Do Not Disturb features enabled so you aren't getting notifications at all. SMS and Calls don't depend on push and neither do many other kinds of notifications. Push is only involved in getting notifications from internet-based services like Signal.

Push means keeping open a connection to the server so it can push data including notifications. Most apps rely on Play services for this but many have fallbacks to their own servers. Only apps without a fallback will be missing support for push-based notifications on GrapheneOS.

Sounds more like you have notifications completely disabled via Do Not Disturb or the other notification settings. Try looking at those settings to see if something has gotten changed. A factory reset would fix it but you won't know what went wrong and it could happen again.
