I think the main question I have left is what treatments, if any, there are to reduce the impact of the mutations on the various diseases (like CVD) that they are associated with

Most of the described variants are associated with elevated homocysteine. Big debates about whether lowering with b vitamin supplementation is beneficial . Most meta analyses are null but recently reinvigorated by recent CSPPT

CSPPT randomized to folic acid, was unique for a number of reasons; only Hcy lowering trial with stroke as primary outcome, and done in China where there's no mandaotey folic acid fortification of food supply so lower background Intake

In their exploratory analyses though, MTHFR C677T subgroup didn't see a reduced risk from having higher folate status. No strong evidence of an effect in this group.

In general, there's little evidence that common MTHFR variants increase needs beyond the current DRI level. A lot of the claims online aren't supported by evidence.

Trial evidence to date has largely focused on MTHFR/Homocysteine/CVD link though - other outcomes it becomes even wonkier. Common variants also increase risk of NTDs but a lot of variance still unexplained, unclear who at risk which is why we fortify food supply.

Brilliant, thanks for the summary! I think that my relative who's been told about MTHFR may have been...misled. The claims were more along the lines of the online unsupported stuff
