Having worked with a few systems, I'd say it depends on who you are designing the software for (i.e. practitioners, patients, researchers)

I'd say the biggest single factor is inertia, from both patients and clinicians. EHR scare everyone regardless of profession!

Ah...very true...if the system doesn't "speak" to the person it is intended for, then the system will not achieve its objective..
