What is the Cohen’s d for a vaccine?

It doesn't work well for discrete effect sizes, but if you use the standard conversion formula the Salk trial (i.e., for a pretty effective vaccine that near eradicated polio) was d = 0.70 or so. d = 3.8 is nearly always a computational error or inappropriate formula
