Should think that would just create confusion and make the analysis even less precise. You need a clear definition of medical error to get any meaning.

@GidMK@BogochIsaac@gorskon@SharkawyMD Not errors but I see a growing trend in the 'hygiene hypothesis'. Parents saying since <12 y o kids were isolating over the past 18 months they're more susceptible to covid (immature immune sys). I think info needs to be pushed out. -
A fundamental misunderstanding about the hygeine hypothesis too, that unfortunately is very common. It’s about exposure to allergens, parasitosis and gi bugs - not viral infections. Viral infections dont attune your immune system, often as not, they attack it to gain infectivity.

Medical errors occur, and when they do and are known, hospitals review what happened to see if was an unavoidable individual or a systems failure.

And change things when possible, to reduce or eliminate sources of error, such as introducing checklists, or having the doctor/nurse repeat back what they are told to decrease the chance something was misheard.
