One thing I've learned from all this fraud assessment is that it is a VERY BAD SIGN if a study reports 100% follow-up and retention. In real studies, at least one person is always lost to follow-up for some reason

YES and the bigger the study, the weirder things get. If no one anticipates the weird then data systems can't capture the full range of happenings during the trial, and you can't report it.
