The problem with ivermectin is that it's a potentially viable treatment and most rational scientists dearly wish it would work but the evidence currently just isn't that great for Covid-19 🤷‍♂️ #WorldIvermectinDay

The real issue is that there's a certain segment of conspiracist weirdos who won't accept anything other than outspoken advocacy for the drug no matter how weak the evidence might be

We will know quite soon whether ivermectin works for COVID-19, and I sincerely hope it does, but pretending the evidence is high-quality when it isn't just makes you seem obsessed not scientific

What is the reason people thought it would work? Based on its MOA, I’m surprised people generated this hypothesis.

it has been shown to inhibit viral replication in the past, most likely by inhibiting importins (completely different MOA to its anti-parasite action), and high conc inhibits SARS-CoV-2 in VERO cells in tissue culture, but *not* human epithelial cells, tricky pharmacokinetics too

Correct. We wish Hydroxychloroquine, Aciclovir, Oseltamivir, or many other anti-virals would work against covid too. But there just is no evidence to support these suggested treatments.

nothing wrong with continuing some RCTs on it but I think we all need to reset our priors to it won't work. When the initial hype claims huge effect size and rigorous studies find no effect, the problem is not sample size or that you didn't test the right combinations

the most plausible study finding a benefit had an effect size comparable to dexamethasone I think? But it had to be administered earlier so it's less useful than dex. And with way more people investigating ivermectin, this finding is way more likely to be a fluke

Why has nobody suggested leeches? The evidence is there.

It's understandable if doctors treating patients (before a vaccine was available) thought they noticed a positive effect w/ IVM. If you listen to their stories, they tried everything in their toolbox, because the whole world was desperate to save lives.
