Low-energy sweeteners & body weight: a citation network analysis https://nutrition.bmj.com/content/early/2021/03/31/bmjnph-2020-000210 … "...inconsistent & selective use of the available evidence may account for the diversity of conclusions in reviews on LES & BW." At least for beverages, artificial sweetener (eg, diet soda) use is so prone to reverse causation bias. It is a "low hanging fruit" swap to make when attempting wt. loss and relatively sustainable. IMO, not likely to be causally related to wt gain.https://pubmed.ncbi.nlm.nih.gov/31582428/

Overall I think it's pretty clear. There's a positive correlation between artificial sweeteners & BMI in observational research, but they're generally associated with weight loss (although only a little) in RCTs. That points to reverse causation.

The challenge is that the two or three best RCTs, which found the biggest benefit for sweeteners, were also industry funded, so there's some worry about publication bias. That being said, no good reason to say they cause weight gain, maybe just not so much loss

When I wrote about it, I excluded industry-funded studies. You're left with a few RCTs showing either nothing or a small benefit for artificial sweeteners. But the important part is that there's just about zero evidence for harm, so why the hell not?

Absolutely agree. And I don't think that we even need to exclude industry studies - they're well done research - just know that in all likelihood there are a couple of unpublished pieces of research showing a smaller benefit

I think that's true. I wanted to make the point that even when you do exclude those, you're still left with evidence showing a small benefit and no harm. Overall, I think discouraging artificial sweeteners is borderline malfeasance on the part of the nutrition community.
